Job summary

Enhabit Home Health & Hospice is seeking a scheduler to assign patients to field clinicians and support care coordination. The role requires an LPN license, CPR, and 1+ year of clinical healthcare experience.

You will communicate with field staff, patients, physicians, and caregivers to ensure continuity of care. Applicants should have strong communication skills, proficiency with Microsoft Office, and a valid state driver license.

Qualifications

• High school diploma or equivalent is required.
• LPN license and valid state clinical license are required.
• CPR certification is required.
• Minimum 1 year of clinical healthcare experience.
• Must have and maintain a valid state driver license.
• Automobile liability insurance required.
• Basic technology skills including mobile devices.

Responsibilities

• Schedule patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other providers to coordinate care.
• Maintain care coordination and continuity of care.
• Manage on-call notebook and hospitalization logs to improve communication.
