Job summary

Enhabit Home Health & Hospice in Texas seeks a scheduling coordinator to align patients with field clinicians and ensure seamless care coordination. You will partner with field staff, physicians, referral sources and caregivers to maintain continuity of care.

The role requires an LPN/LVN license, CPR, and at least one year of clinical experience, plus strong technology and communication skills. In-office attendance is essential to support supervision and teamwork.

Qualifications

• High School Diploma or equivalent is required.
• Licensed Practical or Vocational Nurse license required.
• Valid clinical license per state rules.
• Minimum 1 year of clinical healthcare experience.
• CPR certification required.
• At least 1 year of home health, hospice, or pediatric experience within last 24 months with staffing/scheduling exposure.
• Valid driver license in state of residence.
• Automobile liability insurance.
• Basic technology skills including mobile devices.

Responsibilities

• Schedule patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to improve communication among stakeholders.
