Job summary

Enhabit Home Health & Hospice is seeking a nurse scheduler to coordinate patient appointments with field clinicians and ensure seamless communication among care teams. You will support scheduling, on-call processes, and hospital logs.

The role requires an LPN/LVN license, CPR certification, and a valid driver license, with experience in home health or hospice preferred. Strong tech skills and English proficiency are essential.

Qualifications

• High School Diploma or equivalent is required.
• Licensed practical or vocational nurse license is required.
• Valid state driver license is required.
• CPR certification is required; home health/hospice experience preferred.

Responsibilities

• Schedule patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders.
