Job summary

Enhabit Home Health & Hospice seeks a licensed practical nurse to coordinate patient scheduling with field clinicians, ensuring seamless home-based care. You will communicate across clinicians, physicians, and caregivers to maintain continuity of care and manage on-call and hospitalization logs.

The role emphasizes in-office attendance, strong organizational skills, and proficient use of technology and scheduling tools to support optimal patient outcomes.

Qualifications

• High School Diploma or equivalent required.
• LPN license and valid clinical license per state rules.
• At least 1 year of clinical healthcare experience.
• CPR certification required.
• 1 year of home health, hospice, or pediatric experience preferred in last 24 months.
• Valid state driver license and auto liability insurance required.
• Basic technology skills including mobile device operation.
• Preferred: experience with Homecare Homebase (HCHB).
• Preferred: bilingual English/Spanish communication ability.
• Must be able to travel and maintain transportation.
• Strong oral and written communication and active listening.

Responsibilities

• Schedule patients for field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to ensure care continuity.
• Manage on-call notebook and hospitalization logs to improve stakeholder communication.
