Job summary

Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to link patients with field clinicians and ensure care coordination across teams. You will collaborate with field staff, physicians, and caregivers to maintain continuity of care and timely communications.

Applicants should have a high school diploma, an active LPN/LVN license, CPR, and at least one year of clinical healthcare experience. Domestic travel and a valid driver's license are required to support home-based services.

Qualifications

• High School Diploma or equivalent.
• LPN/LVN license and valid clinical license.
• CPR certification.
• Minimum 1 year of clinical experience in healthcare.
• Valid state driver license.
• Automobile liability insurance as required by law.
• Basic technology skills including mobile devices.
• Preferred: Homecare Homebase (HCHB) experience.
• Preferred bilingual English/Spanish communication ability.
• In-office attendance is essential (no remote work).

Responsibilities

• Schedule patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and service providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to improve stakeholder communication.
