Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to assign patients to field clinicians and coordinate continuity of care.

The role requires an LPN license, CPR certification, at least one year of clinical experience, and strong communication and technology skills; in-office attendance is essential.

You will collaborate with patients, physicians, referral sources, and caregivers to optimize staffing, document on-call notes, and support excellent patient outcomes.

Formación

• Must have High School Diploma or equivalent.
• LPN/LVN license with valid clinical license.
• Minimum 1 year clinical experience in healthcare.
• CPR certification required.
• At least 1 year of home health, hospice, or pediatric experience in the last 24 months.

Responsabilidades

• Schedule patients to field clinicians.
• Coordinate with patients, physicians, referral sources, and caregivers to maintain care continuity.
• Manage on-call notebook and hospitalization logs to enhance communication among stakeholders.
