Ideal candidates have a nursing license (LPN), CPR certification, and at least one year of clinical healthcare experience, with strong technology skills and a valid driver license.

Qualifications

• High School Diploma or equivalent is required.
• LPN license is required and must be maintained.
• CPR certification is required per position.
• Minimum 1 year of clinical healthcare experience.
• Valid driver’s license and automobile liability insurance required.
• Basic technology skills including mobile device operation.

Responsabilités

• Schedule patients to field clinicians and ensure proper care coordination.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers.
• Maintain the on-call notebook and hospitalization logs to support effective communication.
