Enhabit Home Health & Hospice is seeking a scheduling coordinator to arrange patient appointments with field clinicians and ensure seamless care coordination across team members. You will communicate with patients, physicians, and caregivers to maintain continuity of care and manage on-call logistics.

Ideal candidates have an LPN/LVN license or higher, CPR certification, and at least one year of clinical experience in home health, hospice, or pediatric settings. In-office attendance is essential.

Qualifications

• Requires High School Diploma or equivalent.
• LPN/LVN license required.
• Valid clinical license per state rules.
• Minimum 1 year of clinical healthcare experience.
• CPR certification required.
• 1 year of home health, hospice, or pediatric experience preferred.

Responsabilités

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, caregivers, and service providers for care coordination.
• Maintain the on-call notebook and hospitalization logs to improve stakeholder communication.
