Enhabit Home Health & Hospice is seeking an organized, detail-oriented scheduler to coordinate patient appointments with field clinicians and support care continuity. The role requires collaboration with patients, physicians, referrals, and caregivers to ensure timely and quality home health services.

The ideal candidate will have an LPN/LVN license or equivalent, at least one year of clinical healthcare experience, CPR certification, and strong technology skills.

Qualifications

• High School Diploma or equivalent required.
• Licensed Practical or Vocational Nurse with valid license.
• Minimum of one year of clinical healthcare experience.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• 1 year of home health, hospice, or pediatric experience preferred.
• Must have basic technology skills and operate a mobile device.

Responsabilités

• Schedule patients to field clinicians and coordinate care.
• Communicate with staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care continuity.
• Maintain on-call notebook and hospitalization logs to enhance communication among stakeholders.
