Job summary

Enhabit Home Health & Hospice is seeking a scheduling-focused clinical coordinator. You will arrange patient appointments for field clinicians, coordinate with field staff, patients, physicians, referral sources, and caregivers to ensure seamless care coordination and continuity of care.

The role requires an LPN/LVN license, a high school diploma, CPR, a valid driver’s license, and at least one year of clinical experience; familiarity with scheduling and technology; bilingual English/Spanish is

Qualifications

• High School Diploma or equivalent required.
• Licensed practical or vocational nurse license required.
• Valid clinical license per state requirements to practice.
• Minimum one year of clinical healthcare experience required.
• CPR certification required.
• Minimum one year of home health, hospice, or pediatric experience within last 24 months with staffing and scheduling exposure.
• Valid state driver license in current residence; automobile liability insurance required.
• Basic technology skills including operation of a mobile device.
• Preferred bilingual English/Spanish communication.
• In-office attendance essential for Enhabit objectives (remote work not available except for designated roles).

Responsibilities

• Schedule patients to field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other service providers to maintain care coordination.
• Manage the on-call notebook and hospitalization logs to improve stakeholder communication.
