Job summary

Enhabit Home Health & Hospice in Florida is seeking a Scheduling Coordinator to support field clinicians and care coordination. You will schedule patients, communicate with patients, staff, physicians and referral sources, and maintain on-call and hospitalization logs to ensure continuity of care.

The ideal candidate has healthcare experience, a valid driver license, CPR certification, and proficiency with Microsoft Office.

Qualifications

• High School Diploma or equivalent required.
• LPN license required; valid state clinical license required.
• CPR certification required.
• 1 year clinical healthcare experience required or 1 year home health/hospice/pediatric experience in last 24 months.
• Valid state driver license and automobile liability insurance required.
• Basic technology skills; MS Office (Word & Excel) required.
• Preferred bilingual English/Spanish communication.
• In-office attendance is essential; no remote work.

Responsibilities

• Schedule patients to field clinicians and coordinate care.
• Communicate with field staff, patients, physicians, referral sources, caregivers and providers to maintain care coordination.
• Manage on-call notebook and hospitalization logs to enhance stakeholder communication.
