Job summary

Enhabit Home Health & Hospice in Florida seeks a scheduling coordinator to align patient care with field clinicians and ensure seamless care coordination. You will communicate with patients, clinicians, physicians and referral sources while managing on-call logs to support timely patient care.

Qualifications include HD diploma, LPN license, CPR, and at least one year in clinical healthcare; bilingual preferred. In-office attendance is essential to maintain teamwork and service excellence.

Qualifications

• High School Diploma or equivalent required.
• LPN/VN license required per state regulations.
• CPR certification required.
• Minimum of 1 year of clinical healthcare experience.
• Valid state driver's license and auto liability insurance.
• In-office attendance essential to meet Enhabit objectives.

Responsibilities

• Schedule patients to field clinicians and coordinate with patients, physicians, and referral sources.
• Maintain on-call notebook and hospitalization logs to improve care coordination.
