Job summary

Qualifications

Responsibilities

• Schedule patients to branch field clinicians.
• Communicate with field staff, patients, physicians, referral sources, caregivers and other service providers to maintain care coordination and continuity.
• Manage on-call notebook and hospitalization logs to enhance communication among stakeholders.
