Enhabit Home Health & Hospice in Jacksonville, FL is seeking a Scheduler to coordinate patient visits with field clinicians and care teams. You will streamline scheduling and communication to ensure seamless continuity of care.

The role requires strong organizational skills, reliable transportation, and comfort with technology, including mobile devices and Office apps. In this position, in-office presence is essential to support staff and patient needs.

Qualifications

• High School Diploma or equivalent is required.
• Licensed Practical or Vocational Nurse (LPN/LVN) license is required.
• Valid clinical license per state requirements to practice.
• CPR certification is required.
• At least 1 year of clinical healthcare experience is required.
• Home health, hospice, or pediatric experience within last 24 months is preferred.
• Valid state driver license is required.
• Automobile liability insurance is required.
• Basic technology skills including a mobile device are required.
• Bilingual (English/Spanish) is preferred.
• Dependable transportation in good condition is required.
• Oral and written communication and active listening skills are essential.

Responsibilities

• Schedule patients to field clinicians and coordinate care delivery.
• Communicate with field staff, patients, physicians, referral sources, and caregivers to maintain care coordination.
• Manage on-call notebook and hospitalization logs to enhance stakeholder communication.
