Job summary

Enhabit Home Health & Hospice is seeking a scheduling coordinator to align patients with field clinicians and ensure seamless care coordination. You will communicate with field staff, patients, physicians, referral sources, and caregivers to support high-quality outcomes.

Responsibilities include managing on-call and hospitalization logs to enhance communication among stakeholders, with essential in-office attendance to maintain supervision and teamwork.

Qualifications

• High School Diploma or equivalent required.
• Licensed practical or vocational nurse (LPN/LVN) required.
• Valid state clinical license required to practice.
• CPR certification required by position.
• At least 1 year of clinical healthcare experience; home health/hospice/pediatric experience preferred within last 24 months.

Responsibilities

• Schedule patients to field clinicians.
• Coordinate with field staff, patients, physicians, referral sources, caregivers, and other service providers to ensure proper care coordination.
• Maintain on-call notebook and hospitalization logs to improve stakeholder communication.
