Job summary

Enhabit Home Health & Hospice is seeking a Scheduling Coordinator to support care coordination by routing patients to branch field clinicians and maintaining care continuity. You will collaborate with field staff, physicians and referral sources to ensure seamless coordination of care.

Responsibilities include managing the on-call notebook and hospitalization logs, and ensuring timely communication across the care team to maintain high quality home-based services.

Qualifications

• Must possess a high school diploma or equivalent.
• Must either 1) be a licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting; or 2) have at least one year of home health, hospice, or pediatric experience within the last 24 months, and have a demonstrated understanding of staffing and scheduling requirements related to home care services.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Responsibilities

• Schedule patients to branch field clinicians. Coordinate care to maintain proper continuity of care across the team.
