Job summary

Enhabit Home Health & Hospice is seeking a scheduling coordinator to optimize patient care across field clinicians. You will coordinate with field staff, patients, physicians, referral sources, caregivers and other service providers to maintain seamless care coordination and continuity.

The role requires a valid practical nurse license, CPR certification, and at least one year of clinical experience in healthcare, with in-office attendance essential to support supervision, teamwork and timely

Qualifications

• High school diploma or equivalent is required.
• Licensed practical or vocational nurse (LPN/LVN) license is required and must be maintained.
• Valid clinical license as per state requirements.
• CPR certification required.
• Minimum of one year of clinical healthcare experience.
• Must have a valid state driver's license and automobile liability insurance.

Responsibilities

• Schedule patients to field clinicians to ensure timely care.
• Coordinate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain care continuity.
• Manage the on-call notebook and hospitalization logs to improve communication.
