Job summary

Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes across the Dallas–Fort Worth metro area. You’ll perform in-home assessments, develop care plans, and coordinate with physicians and case managers.

Ideal candidates have a current Texas RN license, 1+ year nursing experience (home health preferred), strong clinical skills, reliable transportation, and comfort with EMR.

Qualifications

• Current RN license in Texas.
• 1+ year nursing experience (Home Health preferred).
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate effectively with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation in accordance with regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families
