Job summary

Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Registered Nurse (RN) to provide skilled nursing care to patients in their homes across the Dallas–Fort Worth Metroplex. The ideal candidate is reliable, organized, and confident in clinical decisions, dedicated to helping patients meet health goals.

You will conduct in-home assessments, develop care plans, perform wound care and education, and collaborate with physicians and care teams.

Qualifications

• Current RN license in Texas.
• 1+ year of nursing experience; Home Health preferred but not required.
• Strong clinical assessment and critical-thinking skills.
• Comfortable using EMR/mobile charting and maintaining documentation.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation per regulatory requirements.
• Participate in on-call rotation as needed.
• Build trust with patients and families.
