Job summary
Kona Medical Consulting and The Home Health Team is actively seeking a Home Health Field Registered Nurse (RN) to provide high-quality skilled nursing care to patients in their homes in the Dallas–Fort Worth area. The ideal candidate is reliable, organized, and confident in clinical decision-making, passionate about helping patients reach their health goals.

Qualifications
- Current Texas RN license.
- 1+ year nursing experience; home health preferred.
- Strong clinical assessment and critical-thinking skills.
- Reliable transportation with valid driver’s license and auto insurance.
- Experience with EMR/mobile charting.
Responsibilities
- Perform in-home patient assessments and skilled nursing visits.
- Develop and update individualized care plans.
- Perform wound care, disease management, medication administration, and patient education.
- Communicate with physicians, case managers, and interdisciplinary teams.
- Complete accurate, timely documentation per regulatory requirements.
- Participate in on-call rotation as needed.
- Build trusting relationships with patients and families.
