Job summary

Kona Medical Consulting and The Home Health Team seeks a Home Health Field Registered Nurse (RN) to serve patients in the Dallas–Fort Worth Metroplex. You will perform in-home visits, assess needs, and develop care plans that support safe, effective, and compassionate care in patients' homes.

Ideal candidates have an active Texas RN license, 1+ year of nursing experience (Home Health preferred), and strong clinical judgment.

Qualifications

• Current Texas RN license.
• 1+ year nursing experience; Home Health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation, valid driver’s license, and proof of auto insurance.
• Experience with EMR/mobile charting.
• Patient-focused, compassionate professional.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Perform wound care, disease management, medication administration, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete accurate, timely documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
