Job summary

Kona Medical Consulting is seeking a Home Health Field Registered Nurse (RN) to deliver skilled nursing in patients' homes across the Dallas-Fort Worth Metroplex. The role emphasizes clinical decision-making, organization, and compassionate care to help patients achieve health goals.

The ideal candidate will possess an active Texas RN license, at least 1 year of nursing experience (home health preferred), strong assessment and communication skills, and be comfortable with EMR/charting.

Qualifications

• Current Texas RN license required.
• 1+ year nursing experience; home health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation and valid driver's license.
• Familiarity with EMR/mobile charting.
• Compassionate, professional, and patient-focused care.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Wound care, disease management, medications, and patient education.
• Communicate with physicians, case managers, and interdisciplinary teams.
• Document accurately and timely per regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
