Job summary

Kona Medical Consulting and The Home Health Team seek a skilled Home Health Field Registered Nurse (RN) to deliver high-quality care in patients' homes across the Dallas-Fort Worth Metroplex. The ideal candidate will have Texas RN licensure, at least 1 year of nursing experience, and strong clinical assessment skills.

You will perform in-home assessments, develop care plans, provide wound care and education, and document meticulously. Reliable transportation is required.

Qualifications

• Active Texas RN license required.
• At least 1 year of nursing experience; home health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation with valid driver’s license and auto insurance.
• Experience using EMR/mobile charting.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Provide wound care, disease management, medications, and patient education.
• Communicate effectively with physicians, case managers, and teams.
• Complete accurate, timely documentation per regulations.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
