Kona Medical Consulting and The Home Health Team is seeking a Home Health Field Registered Nurse (RN) to deliver high-quality skilled nursing care in patients’ homes within the Dallas-Fort Worth Metroplex. The role emphasizes reliable clinical decision-making and patient-centered care.

Ideal candidates have Texas RN licensure, 1+ year of nursing experience (home health preferred), strong assessment skills, and comfort with EMR/mobile charting.

Qualifications

• Texas RN license is required.
• Minimum 1 year of nursing experience; home health preferred.
• Strong clinical assessment and critical-thinking skills required.
• Reliable transportation, valid driver’s license, and auto insurance.
• Comfortable using EMR/mobile charting.
• Compassionate, professional, and patient-focused.

Responsibilities

• Conduct in-home patient assessments and skilled nursing visits.
• Develop and update individualized care plans.
• Provide wound care, disease management, medications, and patient education.
• Communicate effectively with physicians, case managers, and interdisciplinary teams.
• Complete documentation in accordance with regulatory requirements.
• Participate in on-call rotation as needed.
• Build trusting relationships with patients and families.
