Job summary
Kona Medical Consulting and The Home Health Team is actively seeking a Home Health Field Registered Nurse (RN) to provide skilled nursing care in patients' homes within the Dallas–Fort Worth Metroplex. The ideal candidate will have a Texas RN license, 1+ year of nursing experience, strong clinical assessment skills, and comfort with EMR/mobile charting.
Travel is required, with mileage reimbursement and flexible scheduling to support patient-centered care.
Qualifications
- Texas RN license in good standing.
- 1+ year nursing experience; home health preferred.
- Strong clinical assessment and critical-thinking skills.
- Reliable transportation and valid driver's license.
Responsibilities
- Perform in-home patient assessments and skilled nursing visits.
- Develop and update individualized care plans.
- Perform wound care, disease management, medication administration, and patient education.
- Communicate with physicians, case managers, and interdisciplinary teams.
- Complete accurate, timely documentation per regulatory requirements.
- Participate in on-call rotation as needed.
- Build trusting relationships with patients and families.
