Job summary

Kona Medical Consulting and The Home Health Team are seeking a Home Health Field Registered Nurse (RN) to deliver skilled nursing care to patients in their homes across the Dallas-Fort Worth area.

The ideal candidate will have Texas RN licensure, 1+ year nursing experience (home health preferred), strong clinical assessment skills, and reliable transportation. You will work with physicians and care teams, document accurately, and participate in on-call rotations.

Qualifications

• Current Texas RN license.
• At least 1 year of nursing experience; home health preferred.
• Strong clinical assessment and critical-thinking skills.
• Reliable transportation and valid driver's license with auto insurance.
• Experience with EMR/mobile charting preferred.
• Compassionate, professional, patient-focused.

Responsibilities

• Perform in-home patient assessments and skilled nursing visits
• Develop and update individualized care plans
• Perform wound care, disease management, medication administration, and patient education
• Communicate with physicians, case managers, and interdisciplinary teams
• Complete accurate, timely documentation per regulatory requirements
• Participate in on-call rotation as needed
• Build trusting relationships with patients and families
