Kona Medical Consulting and The Home Health Team in Dallas–Fort Worth seeks a dedicated Home Health Field Registered Nurse (RN) to deliver high-quality skilled nursing care in patients' homes. The ideal candidate is reliable, organized, confident in clinical decision-making, and passionate about helping patients reach their health goals.

Must hold a Texas RN license with 1+ year of nursing experience; home health experience preferred.

Qualifikationen

Texas RN license is required.
1+ year of nursing experience; home health preferred.
Strong clinical assessment and critical thinking skills.
Reliable transportation, valid driver’s license, and auto insurance.
Familiar with EMR/mobile charting.
Compassionate, professional and patient-focused.

Aufgaben

Perform in-home patient assessments and skilled nursing visits.
Develop and update individualized care plans.
Perform wound care, disease management, medication administration, and patient education.
Communicate effectively with physicians, case managers, and interdisciplinary teams.
Complete accurate, timely documentation per regulatory requirements.
Participate in on-call rotation as needed.
Build trusting relationships with patients and families.
