Job summary
Kona Medical Consulting and The Home Health Team in Dallas–Fort Worth seeks a dedicated Home Health Field Registered Nurse (RN) to deliver high-quality skilled nursing care in patients' homes. The ideal candidate is reliable, organized, confident in clinical decision-making, and passionate about helping patients reach their health goals.
Must hold a Texas RN license with 1+ year of nursing experience; home health experience preferred.
Qualifications
- Texas RN license is required.
- 1+ year of nursing experience; home health preferred.
- Strong clinical assessment and critical thinking skills.
- Reliable transportation, valid driver’s license, and auto insurance.
- Familiar with EMR/mobile charting.
- Compassionate, professional and patient-focused.
Responsibilities
- Perform in-home patient assessments and skilled nursing visits.
- Develop and update individualized care plans.
- Perform wound care, disease management, medication administration, and patient education.
- Communicate effectively with physicians, case managers, and interdisciplinary teams.
- Complete accurate, timely documentation per regulatory requirements.
- Participate in on-call rotation as needed.
- Build trusting relationships with patients and families.
