Job summary

Enhabit Home Health & Hospice is seeking a qualified nursing or physical therapy professional to review and approve patient information during start of care and recertifications, ensuring regulatory compliance.

Role collaborates with branch leadership to educate LPs and maintain strong referral relationships, with emphasis on documentation accuracy and quality patient care across home health services.

Qualifications

• The candidate must be a graduate with an active professional license (nursing or physical therapy).
• Minimum one year of experience in a Medicare home health or hospice setting.
• Strong documentation and communication skills required.
• Valid driver license and automobile liability insurance required.
• Proficient with basic technology and mobile devices.

Responsibilities

• Review and approve all patient information submitted by LP during start of care or re-certification.
• Partner with branch leadership to educate LP staff on documentation requirements.
• Provide one‑on‑one training when needed to ensure regulatory compliance.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
