Job summary

Enhabit Home Health & Hospice is seeking a licensed nurse or physical therapist to review patient information and ensure accurate start-of-care documentation. You will train LPs and ensure regulatory compliance while promoting high-quality patient care.

The role requires active CPR certification, a valid state license, and in-person attendance at the office location. Strong technology skills and familiarity with Homecare software are essential.

Qualifications

• Graduate from an approved school of professional nursing or physical therapy.
• Valid clinical license per state requirements to practice.
• Minimum 1 year of experience; Medicare home health or hospice preferred.
• CPR certification and a valid state driver license with automobile liability insurance.
• Strong technology skills including Microsoft Office and mobile device use.

Responsibilities

• Review and approve patient information submitted by LPs during start of care, recertification, resumption of care, or evaluation visits.
• Provide ongoing education and training to branch LPs to ensure staff understand documentation requirements and regulatory standards.
• Meet one-on-one with LPs to provide in-depth training when necessary.
• Promote quality patient care and maintain relationships with referral sources, patients, caregivers and employees.
