Job summary

UnitedHealth Group, through Amedisys, seeks a Per Diem Physical Therapist Assistant to provide skilled care in patients' homes under a licensed PT. You'll implement treatment plans to improve mobility, strength, balance, and independence while promoting safety.

Responsibilities include documenting progress, coordinating with the care team, and supporting recovery and quality of life. A CAPTE-accredited PTA program graduate with an active PTA license and CPR certification is required; mentorship

Qualifications

• Graduate from a CAPTE-accredited PTA educational program.
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ years of related PTA experience for home health care.
• If less than one year of experience, approval from regional clinical leaders and mentoring/residency may be required.

Responsibilities

• Provides PTA services under supervision of a licensed PT as per plan of care.
• Observes and reports patient response to treatment.
• Documents supervisory activities in medical records.
• Monitors patient progress and adjusts care as needed.
• Participates in care conferences and quality improvement activities.
• Maintains confidentiality of patient information.
• Coordinates follow-up care with admitting agency and community resources.
• Performs other duties as assigned.
