Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist Assistant to provide skilled PTA services in patients’ homes under the supervision of a licensed Physical Therapist. The role focuses on improving mobility, strength, balance, and independence while promoting safety in the home setting.

You will work with patients, caregivers, and the interdisciplinary team to support recovery and quality of life. A CAPTE-accredited PTA program, active state license, and CPR certification are required.

Qualifications

• Graduate from a CAPTE-accredited Physical Therapy Assistant program.
• Current license to practice as a PTA in the state of assignment.
• Current CPR certification.
• 1+ year of PTA experience in home health care.
• If less than 1 year of experience, mentoring and/or residency program approval is required.

Responsibilities

• Provides physical therapy services and interventions under the supervision of a licensed PT and per the plan of care.
• Observes and reports patient response to treatment and any condition changes to supervising PT and clinical staff.
• Provides resources for future needs and promotes wellness and prevention.
• Documents supervisory activities in the patient's medical record.
• Monitors and documents patient response to PT intervention.
• Participates in patient care conferences, in-services, chart audits and quality improvement activities.
• Maintains confidentiality of PT-client information.
• Makes follow-up care recommendations in coordination with the admitting agency and other care providers.
• Performs other duties as assigned.
