Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist Assistant to provide skilled PT services in patients' homes under the supervision of a licensed Physical Therapist. You will implement individualized treatment plans to improve mobility, strength, balance, and independence, while coordinating with patients, caregivers, and the interdisciplinary team.

The role emphasizes safety, documentation, and collaboration with the care team to support recovery and enhance quality of life at home.

Qualifications

• Current license to practice as a physical therapy assistant (state-specific).
• Graduation from CAPTE-accredited PTA program.
• CPR certification is current.
• One year related PTA experience in home health or prep for mentoring if less than one year.

Responsibilities

• Provides PT services under supervision of a licensed PT.
• Observes and reports patient response to treatment.
• Promotes wellness and prevention.
• Documents supervisory activities in medical record.
• Monitors patient response to PT interventions.
• Participates in care conferences and quality improvement activities.
• Maintains confidentiality of patient information.
• Makes follow-up care recommendations with coordinating agencies.
• Performs other duties as assigned.
