Amedisys, part of the Optum family, is seeking a Per Diem Physical Therapist Assistant to provide skilled in-home care under a supervising Physical Therapist. You will implement individualized treatment plans to improve mobility and independence, collaborate with patients, caregivers and the care team, and support recovery at home.

Per diem roles require CAPTE-accredited training, state PTA licensure and CPR certification, with 1+ year home-health experience or eligibility for mentoring.

Qualifications

• Graduate from an accredited physical therapy assistant program (CAPTE).
• Current license to practice as a PTA in the assigned state.
• Current CPR certification.
• 1+ year PTA experience for home health care; mentoring if <1 year may be required.

Responsabilités

• Provides PT services under the supervision of a licensed PT according to the plan of care.
• Observes and records patient responses to treatment and reports changes.
• Promotes wellness, prevention, and home safety.
• Documents supervisory activities in the patient record.
• Monitors patient progress and documents responses to therapy.
• Participates in care conferences, trainings, and quality improvement activities.
• Maintains patient information confidentiality.
• Coordinates follow-up care with sending/receiving agencies.
• Performs additional duties as assigned.
