Job summary

UnitedHealth Group is seeking a Physical Therapist Assistant to provide skilled home health care under the supervision of a licensed physical therapist. You will implement individualized treatment plans to improve mobility, balance, and independence in patients' homes, while promoting safety and caregiver collaboration.

You will work with patients, families and the care team, document progress, and participate in care conferences.

Qualifications

• Graduation from CAPTE-accredited PTA program.
• Current CPR certification.
• Current license to practice as a PTA in the assigned state.
• 1+ year of related PTA experience in home health care.

Responsibilities

• Provide physical therapy services under the supervision of a licensed PT.
• Observe and report patient responses to treatment and changes in condition.
• Promote wellness, prevention and patient safety; document in medical records.
• Coordinate follow-up care with agencies and other care providers.
