Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver’s license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Current CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Evaluates the patient's functional status and physical therapy needs.
• Develops and implements the plan of care.
• Performs interventions to improve and enhance wellbeing.
• Documents progress notes per policy.
• Communicates plan of care to patient and family.
• Visits patients per Plan of Treatment and reports outcomes.
