Job summary

This Physical Therapist Assistant will cover the Englewood, Chatham, and Hyde Park areas of Chicago, providing treatment and patient education as part of the home health team within Optum family. You will implement the plan of care under supervision and coordinate care with the healthcare team.

Responsibilities include active and passive exercises, gait and functional training, and ensuring compliance with state laws and documentation.

Qualifications

• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
• Available to work a weekend rotation.

Responsibilities

• Follow state-specific laws governing provision of physical therapy in home care.
• Follow treatment defined by supervising PT and recommend plan of care changes.
• Instructs and aids patients in active and passive exercise, gait, ADL, and mobility training.
• Observe and report patient responses to treatment to the supervising PT and care team.
• Coordinate care with health care team and educate patient and family about therapy within scope of PT support.
