Optum is seeking a Physical Therapist Assistant to cover Englewood, Chatham and Hyde Park areas in Chicago for IL Home Health. You will implement treatment and provide exercise instruction to improve patient well-being under the supervising PT.

Responsibilities include observing patient responses, coordinating with the care team, and educating patients and families. A weekend rotation is required with a comprehensive benefits package.

Qualificações

• Current, licensed Physical Therapist Assistant in IL.
• Current CPR certification.
• Valid driver's license and access to a vehicle or reliable transportation.
• Able to work a weekend rotation.

Responsabilidades

• Follow supervising PT plan of care and report patient responses.
• Instruct and assist with active & passive exercise, gait, ADL, transfers, safety, and prosthetic training.
• Coordinate care with the healthcare team and educate patient and family.
• Perform procedures only within qualification and training and document changes in plan of care.
