Job summary

Optum is seeking a Physical Therapist Assistant to join our IL Home Health team serving Englewood, Chatham, and Hyde Park in Chicago. You will perform treatment, provide exercise instruction, and educate patients to improve well-being under supervision, following the plan of care.

Primary responsibilities include adhering to state PT laws, instructing patients in exercises, and coordinating care with the healthcare team. Weekend rotation is required and a competitive benefits package is offered.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current CPR certification.

Responsibilities

• Follow state-specific laws governing home PT provision and the defined treatment plan.
• Instruct and aid patients in active and passive exercise, gait, ADL, and transfer activities.
• Observe and report patient responses to treatment to supervising PT and healthcare team.
• Coordinate care with the health care team and educate patient, family, and caregivers on therapy areas.
• Provide care in Englewood, Chatham, and Hyde Park areas as part of IL Home Health.
