Job summary

Optum is seeking a Physical Therapist Assistant to cover Englewood, Chatham and Hyde Park areas of Chicago in home health settings. You will perform treatment and educate patients per the plan of care under supervision, supporting mobility and daily function.

Responsibilities include following state laws, assisting with exercise programs, and coordinating with the care team. Weekend rotation and reliable transportation are required.

Qualifications

• Licensed/registered/credentialed to assist in physical therapy in the state of practice.
• Current CPR certification.
• Valid driver’s license and access to reliable transportation.

Responsibilities

• Follow supervising PT's plan of care and state laws for home health PT involvement.
• Instruct and assist patients with active and passive exercise, gait, and functional activities; ensure patient safety.
• Observe and report patient responses to treatment to supervising PT and care team; coordinate with the health care team.
