Lake Norman Home Health, a part of LHC Group within the Optum family, seeks a Physical Therapist Assistant to perform treatment, provide exercise instruction, and educate patients per plan of care in a home health setting.

Responsibilities include following state PT laws under supervision, assisting with gait/ADL training, coordinating care with the health team, and adapting to diverse home environments while communicating with patients and families.

Qualifications

• Current, licensed Physical Therapist Assistant in North Carolina.
• Valid driver's license and reliable transportation.
• Weekend availability.

Responsabilités

• Follow state laws and the supervising PT plan of care.
• Instruct and aid patients in active/passive exercise and mobility training.
• Record patient progress and coordinate with the care team.
• Educate patient, family, and caregivers on therapy techniques.
