Job summary

At Home Healthcare, part of LHC Group and within the Optum family, is seeking a Physical Therapist Assistant to support patients in home care. You will follow the plan of care, educate patients, and collaborate with the supervising PT to optimize outcomes.

Responsibilities include implementing prescribed exercises, guiding families, and documenting patient progress. Requirements include PTA licensure in the state and weekend availability; valid driver’s license preferred.

Qualifications

• Current, licensed PTA in the state of practice.
• Weekend rotation availability.
• Strong communication with patients and care team.

Responsibilities

• Follow the plan of care defined by the supervising PT.
• Instructs and assists patients with exercises, gait, and functional activities.
• Observes and reports patient progress and changes to the supervising PT.
• Educates patient, family, and care team on therapy within PTA scope.
• Maintain safety and adapt care to home environments.
