Job summary

Qualifications

• Graduated from an accredited physical therapy program as set by CAPTE.
• Current license to practice physical therapy in the assigned state.
• Current CPR certification.
• 1+ years of PT experience in home health or equivalent.
• If less than 1 year, regional mentoring/approval required.

Responsibilities

• Provide skilled PT services to patients in their homes to improve mobility, function and safety.
• Perform reexaminations at 30-day intervals and discharge planning.
• Collaborate with patients, caregivers and care team to support recovery.
