Job summary

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of related PT experience in home health preferred.
• If <1 year, mentoring/residency participation may be required.
• Current CPR certification.
• Driver's license and reliable transportation.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Conduct reexaminations at least every 30 days and adjust interventions as needed.
• Discharge planning and other duties as assigned.
