Job summary

Caretenders, a part of LHC Group within UnitedHealth Group, is seeking a Physical Therapist for Home Health on a per diem basis with at least 30 hours per month. You will assess patients, develop and modify plans of care, and deliver interventions to improve mobility and comfort.

This role requires current state PT licensure and a valid driver's license; CPR certification preferred. Compensation includes per visit points, hourly pay, and benefits where eligible.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Evaluates patient needs and develops a plan of care for home health.
• Administers PT interventions to improve mobility, endurance, and function.
• Documents progress notes and adjusts treatment as needed per plan of care and state guidelines.
• Coordinates with supervising nurse/physician and health care team.
