Job summary

UnitedHealth Group is seeking a Per Diem Physical Therapist Assistant to provide skilled care in patients’ homes under the supervision of a licensed Physical Therapist. You will implement individualized treatment plans to improve mobility and independence, while ensuring safety and quality of life at home.

The role requires CAPTE-accredited PTA training, a current PTA license in the state of assignment, CPR, and eligible for regional mentoring if experience is less than one year.

Qualifications

• Graduate from an accredited physical therapy assistant educational program as set by CAPTE.
• Current license to practice as a PTA in the state you are assigned to work in.
• Driver’s License and access to reliable transportation.
• Current CPR certification.
• One year of related PTA experience for home health care.
• If less than one year of experience, approval from regional clinical leaders and appropriate mentoring.

Responsibilities

• Provides physical therapy services and interventions under the supervision of a physical therapist and in accordance with the physician-approved plan of care.
• Observes, records and reports patient response to treatment and changes in condition to supervising PT and clinical staff.
• Provides resources for future needs and promotes wellness and prevention.
• Documents supervisory activities in the patient’s medical record.
• Monitors and documents patient response to PT interventions.
• Participates in care conferences, in-services, chart audits and quality improvement activities.
• Maintains confidentiality of patient information.
• Recommends follow-up care in coordination with the admitting agency and other providers.
• Performs other duties as assigned.
