Job summary

Qualifications

• Current licensure as a physical therapist in the practicing state(s).
• Graduate of an accredited physical therapy education program recognized by the APTA.
• Within the last five years, two years of experience as a physical therapist relevant to the patient population. CPR certification must be obtained within 60 days.

Responsibilities

• Assist in evaluating patient function and plan of care with goals and interventions.
• Complete initial and ongoing comprehensive assessments at scheduled times.
• Provide interventions and therapies per the plan of care and professional standards.
• Re-evaluate patient status and modify services with physician revision orders as needed.
• Educate patient and family to promote progress and plan for discharge.
• Communicate with interdisciplinary team to coordinate care.
• Prepare timely documentation of care and patient progress.
