Job summary

You will assess, plan and reexamine every 30 days, discharge planning, and collaborate with patients and caregivers. A state PT license, CAPTE-accredited education, and CPR certification are required; 1+ year home health experience preferred.

Qualifications

• Licensed physical therapist in the assigned state.
• Graduate from CAPTE-accredited program.
• 1+ year of PT experience in home health or equivalent.
• If <1 year experience, regional clinical leaders approval and mentoring/residency required.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as needed, and perform discharge planning.
• Other duties as assigned.
