Job summary

UnitedHealth Group is seeking a Physical Therapist to provide skilled physical therapy services to patients in their homes, promoting mobility, restoring function, and improving safety and independence. You will collaborate with patients, caregivers, and an interdisciplinary care team to support recovery and prevent hospitalizations.

Responsibilities include helping patients maintain movement, conducting reexaminations every 30 days, adjusting interventions as needed, and performing discharge

Qualifications

• 1+ years related physical therapy experience for PTs entering home health care.
• Current license to practice physical therapy specific to the state you're assigned to work in.
• Driver's License and access to reliable transportation.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days at minimum), modify interventions as necessary, and perform discharge planning.
• Other duties as assigned.
