U.S. Renal Care seeks a Home Therapy Registered Nurse to train patients on home dialysis and provide ongoing support. You will perform home visits, maintain detailed nursing documentation, and collaborate with physicians to optimize patient care.

The role requires an active RN license, CPR, and experience in dialysis nursing, with on-call rotations and a commitment to safety and quality care in home settings.

Qualifikationen

• Current RN license in applicable state.
• 12 months of clinical nursing experience with 3 months in home dialysis modality.
• CPR certification required prior to patient care.
• Ability to distinguish all primary colors.
• Must complete Home Therapy Nurse Orientation and maintain skills.
• Ability to communicate in English, both verbally and in writing.
• Basic computer skills; proficiency in USRC clinical applications within 90 days of hire.

Aufgaben

• Provide patient training and ongoing support for home dialysis.
• Conduct home visits to assess the patient’s home environment.
• Document nursing services in the EMR with accurate status reflections.
• Coordinate with physician and team on patient care and monitoring.
• Participate in infection control monitoring and reporting.
• On-call nursing duties, nights and weekends on a rotation.
• Ensure timely supply orders and inventory management for patient care.
• Strive to improve patient outcomes through data tracking and CQI activities.
