Job summary

U.S. Renal Care is seeking a Home Therapy Registered Nurse to train patients and care partners in self-care dialysis and provide ongoing support. You will perform home visits, assess environments, document care in the EMR, and communicate findings to the physician and care team.

The role requires current RN licensure, CPR certification, and experience in dialysis. Duties include on-call coverage and coordinating with the clinical team to improve patient outcomes in a patient-centered environment.

Qualifications

• Current RN license in applicable state and active status.
• 12 months of clinical nursing experience with 3 months in home dialysis training.
• CPR certification and ability to communicate effectively in writing and verbally.

Responsibilities

• Train patients and care partners for self-care peritoneal or hemodialysis.
• Conduct home visits to assess home environment and patient needs.
• Document all nursing services in EMR with accuracy and timeliness.
• Collaborate with physicians and care team; monitor patient status and outcomes.
• Coordinate on-call coverage nights and weekends; ensure supplies and inventory are managed.
